memory card turned to read only

I had a memory card. Yesterday i used it. I deleted files from it and copied some files into it. But after sometime, i want it to be empty. For that i used FORMAT option. I clicked on the Format. It did format. It looked empty. But in just 5 seconds, it just disappeared. I removed the memory card and re-inserted into USB port. But it is aking as
((( You need to format the disk in drive F: before you can use it. ))) Then i clicked on the {{{ Format disk }}}. But nothing changed.
Additional information: After i remove the Memory card from CPU, the drive option disappeared.
When i re-inserted it, the drive option is coming. As [[ Removable Disk (F:) ]]. When i click on it, it is showing as ((( please insert a disk into Removable Disk (F:) ))).


Somebody give me a solution.
