Looking to buy a new GPU and Monitor, suggestions please.

I am looking to buy a new one but I have no clue what to look for and it all looks like techno-mumbo-jumbo to me.
I also want to buy a new monitor as people say that using a T.V isn't the best idea.

My specs:

Monitor - GVA 32" 1080p HD LCD T.V (THIS IS WHAT I WANT TO REPLACE)
Motherboard - Gigabyte 970A-D3
CPU - AMD FX-6100 Six-Core
RAM - 8GB
Graphics Card - AMD Radeon HD 6870 (THIS IS WHAT BROKE)
PSU - Antec NeoECO 620c

My budget is around $250...no more than $265.
I want the new GPU to be just as good, if not better than the one that broke
It mainly just needs to work....and have warranty in-case the darn thing breaks like all computer parts do. -_-

I will be using it pretty much just for gaming....Dark Souls 2 is my waifu.
