Hard drive issue (possibly)

Hi everyone! :)

The problem: my laptop is running slow. very slow sometimes.

I have an issue with my computer, and it has been around for quite a long time. It started about half a year ago.
To cut the long story short:
1. An alert popped up, saying:  "high rate of read/write errors. your hard drive may be failing, (backup data etc...)". It was Argus Monitor that showed dis pop-up.
2. I was really worried, started to backup my data.
3. I was still worried, thought it was a software issue (virus, malware, anything), so I made a full wipe on my hard drive.
4. Installed Windows 8.
5. The problem still exists. :(
5.1. The reason it could last half a year without a real solution is because of the random occurance of the problem.
The computer usually works fine, I can do what I want to (browse, learn, or even play games like CoD MW3, so it can, if it wants to. )
6. However, the RESPONSE TIMES are way too high all the time. Instead of 20-50ms, they are usually between 500-3000ms.
I lately saw a response time of 22.000ms! yes, twenty-two seconds!

I have made virus scans (even boot-time), malware removal, defrag, junk files cleaning, repair of drives, chkdisk, etc. These that come to my mind right now.

Please help me, this problem is starting to really annoy me.
(plus I can't live without a computer, I need it for learning for my exams)

My laptop: TOSHIBA Satellite C855-12K, 15.6" LED 1366x768, Pentium B960, DDR3 4GB, Radeon HD7610M, 500GB HDD
